This page contains forms that are useful for our members.  Most of these forms can be found on various pages within the site, but having all forms on one page is sometimes easier for you to find the correct form you need.

Financial Forms

SVBR Expense Reimbursement Form
This is a combined mileage and general expense reimbursement form.  The mileage reimbursement rate is valid for travel until December 31, 2012.

Membership Application
For complete details on becoming a member of SVBR, see our "Become A Member" page. This is the application form required to begin the process of becoming a member.

Real Estate License Change of Information Form
This form is required when a licensed salesperson changes firms or other information on file with the Real Estate Commission.

Transfer Office Form
All applicants transferring firms who have Primary or Secondary REALTOR® membership with SVBR are required to complete this application and submit along with a signed copy of the Vermont Real Estate Commission’s “Change of Information Form”.
